Verifying the Social Implementation of Pre-Disease Health Initiatives at Fujisawa SST

At Fujisawa Sustainable Smart Town (Fujisawa SST), a demonstration project was conducted in collaboration with Kanagawa Prefecture, taking an integrated approach to frailty prevention, dementia care, and the improvement of pre-dementia conditions.

This initiative aims to verify a foundational framework for the social implementation of health promotion from the pre-disease stage in communities facing rapid population aging.

In the frailty prevention program, interventions combining fall prevention exercises and chewing function training were provided to older adults, and changes in physical functions were quantitatively evaluated. The study also examined effects contributing to frailty prevention, such as increased health awareness through physical activity.

In the dementia care program, an experiential initiative was implemented primarily for older adults without dementia, combining community events with the use of a dementia care application. Changes in participants’ understanding of dementia and attitudes toward social participation were evaluated.

In addition, efforts were made to establish a system that continuously visualizes lifestyle habits, cognitive function, and levels of social participation through the use of a personal health record (PHR) application.

By positioning Fujisawa SST as a demonstration field for collaboration among government, industry, and academia, we aim to build a sustainable, community-based model for the social implementation of pre-disease health initiatives.
